You have have both good & bad characters on the same account. The differences are pretty minor after the first few levels, although it's still difficult to go wandering in the opposite alignment's cities (although a few heritage quests need you to do so - usually via the sewer system). The main differences are the races & classes you can pick e.g. a shadowknight can only be bad, paladin only good. Trolls can only be bad unless they do a betrayal quest (SK would become a paladin), etc.
Once you hit level 20 or so, you are out in the wide world, in the same zones & can group and chat freely with anyone (except on PVP servers). There are no problems with mixed alignment guilds either, and 'evil' characters can still get in & out of their Qeynos guildhall.
Edit: You can also freely pass items & cash between your characters via your shared bank slots, regardless of alignment. Buy some big boxes to put in the bank slots and you can share most of your stuff with *all* of the characters on your account/server.

They made it a lot easier over the years, I mean the little things like shared bank slots even between evil and good characters which I believe was not possible in the early days. (I seem to recall this for a reason, someone correct me if I'm wrong plz)
